1.) The address you provide below must match the billing address of the cardholder. If you do not use your exact billing address, your order will not go through and you will receive an error.

If you would like to ship your order to a location other that your billing address you should add the shipping address in the 'Comments' section below.

2.) DO NOT use a slash (/) or dash (-) in-between the Exp. Date. Only use numbers. If your credit card expires in December 2006 use "1206" not 12/06.

Also, do not use any spaces or dashes in-between your credit card numbers.
